Wondering How to Get My ReFills Into the Reason Shop

I 'm just wondering how one goes about getting a ReFill on the Reason Shop. I tried this link...

... but it seems out of date. The Email Form link sends you to a page with "Support" or "Feedback" links and the "We also need you to fill out this contract and send it to us" link doesn't work at all. Any help would be much appreciated, and thanks in advance.
